Invoice for Direct to Pay

For Direct-To-Pay through the Arizona ESA Program:

1. Fill out the following form, so we can generate your invoice

2. An invoice and curriculum will be emailed to you within 24 hours

3. Once you've received your invoice and curriculum, submit them to Class Wallet

4. Once we are notified by Class Wallet, we will pack and ship your box within 3-5 business days.
