Invoice for Direct to Pay

For Direct-To-Pay through the Arizona ESA Program:

1. Fill out the following form, so we can generate your invoice

2. An invoice will be emailed to you within 24 hours

3. Once you've received your invoice, submit it to Class Wallet

4. For 1-3 day shipping, email us the screenshot of the confirmation, otherwise your box will ship once we are notified by ClassWallet 5-12 days after you have submitted your invoice, due to the current manual reviews.
